Key Responsibilities
- Performing basic housekeeping chores, such as vacuuming, doing laundry, dusting and washing dishes
- Grocery shopping and preparing meals, according to clients’ preferences and individual dietary needs and restrictions
- Assisting with daily living tasks, such as bathing, grooming, dressing, toileting and feeding
- Checking patient vital signs, such as heart rate, blood pressure, and body temperature
- Scheduling medical appointments and providing transportation services as needed
- Providing companionship through conversation and activities
- Keeping accurate records of services performed, patients’ health status and hours worked
- Medication reminders
- Monitor health conditions, report changes to supervisors
Required Skills and Qualifications
- Certifications: Active CNA license or state-approved HHA certificate.
- Physical Ability: Ability to lift, transfer, and reposition patients.
- Soft Skills: Compassion, patience, strong communication, and dependability.
- Knowledge: Basic understanding of infection control, safety, and hygiene
Key Responsibilities
- Clinical Care: Administer medications, perform wound care, monitor vital signs,O2saturation, blood pressure, heart rate), and perform injections or blood draws.
- Patient Assessment: Evaluate patient health status regularly, documenting and reporting any significant changes to doctors or RN supervisors.
- Documentation: Maintain accurate, detailed, and timely medical records of all care provided.
- Education: Instruct patients and caregivers on disease management, medication adherence, and safety measures.
- Coordination: Collaborate with a multidisciplinary team (physicians, physical therapists, social workers) to implement and update the plan of care.

Requirements and Qualifications
- Licensure: Active, RN or LPN license in the state of practice.
- Experience: Prior experience in home health, acute care, or skilled nursing preferred.
- Skills: Strong assessment, wound care, and communication skills; ability to work independently.
- Attributes: High empathy, patience, and the ability to remain calm in emergencies.
- Logistics: Reliable transportation and, often, a valid driver’s license
